Partneri u projektu su, uz vode\u0107eg partnera Udruge osoba sa invaliditetom Bubamara, Vinkovci , Caritas \u0160abac i Dom zdravlja dr. Draga Ljo\u010di\u0107 te Dom za starije i nemo\u0107ne osobe Ilok. Projekt \u0107e se provoditi u vremenskom periodu od 01.05.2019.-30.04.2020. godine. Projektne aktivnosti sa hrvatske strane provodi Udruga osoba sa invaliditetom Bubamara Vinkovci i Dom za starije i nemo\u0107ne osobe Ilok na podru\u010dju Vukovarsko-srijemske \u017eupanije.<\/p>\n<p style=\"text-align: justify;\">Glavni cilj projekta je pru\u017eanje zdravstvenih i socijalnih usluga namijenjenih starijim, nemo\u0107nim i osobama s invaliditetom. Projekt se temelji na integriranome modelu skrbi, odnosno me\u0111usektorskoj suradnji koja poti\u010de sveobuhvatnu intervenciju i zbrinjavanje najosjetljivijih skupina u dru\u0161tvu.&nbsp; Kroz projekt \u0107e biti educirano sveukupno 100 zdravstvenih, socijalnih i ostalih djelatnika.<\/p>\n<p style=\"text-align: justify;\">Dom za starije i nemo\u0107ne osobe Ilok \u0107e kroz projekt Support life rekonstruirati te opremiti odjel za njegu bolesnika u terminalnoj fazi bolesti te organizirati mobilni tim koji \u0107e odlaziti u ku\u0107ne posjete najosjetljivijim skupinama dru\u0161tva. Usluge \u0107e pru\u017eati&nbsp; medicinska sestra, dvije gerontodoma\u0107ice, lije\u010dnik op\u0107e prakse,&nbsp; fizioterapeut, psihijatar i ku\u0107ni majstor. Mobilni tim \u0107e kroz projekt pru\u017eati svoje usluge za 120 korisnika prilago\u0111eno njihovim potrebama.<\/p>\n<p style=\"text-align: justify;\">Tako\u0111er \u0107e se nastaviti rad posudionice medicinskih i ortopedskih pomagala koja \u0107e se pro\u0161iriti nabavom dodatnih pomagala sredstvima iz projekta.<\/p>\n<p style=\"text-align: justify;\">Ukupna vrijednost cjelokupnog projekta je 992 437,99\u20ac od \u010dega 85% financira Interreg-IPA CBC HR-RS, a preostalih 15% financiraju projektni partneri.
